Does anyone have an example of an LSAT score jump addendum?

Certain schools require an addendum for an LSAT point differential. I have a basic idea of what I want to say, but I always like taking a look at examples to know that I'm on the right track. I've searched around a bit though and can't seem to find any. Anyone know of any good examples you can link to?
